🩺 What You’ll Do

Perform in-home nursing assessments and create personalized care plans
Provide skilled care: wound care, med management, post-acute follow-up
Supervise and support LPNs, CNAs and HHAs
Coordinate care with families and providers
Use simple electronic charting and stay compliant with regulations
Participate in phone-based on-call rotation (no field visits required)

✅ What You Bring

Active RN license in Minnesota and Wisconsin (or ability to obtain both)
2+ years of clinical experience (home health preferred)
Excellent critical thinking and communication skills
CPR certification and valid driver’s license
TB test or chest x-ray clearance
Comfort using EMR systems and electronic charting
