Seeking a PTA to join our home care team in the Washington County and St Croix County areas.

Responsibilities
  • Make a difference for your clients and their families in their own home!
  • Implementation of all physical therapy modalities to patients
  • All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization
  • Improves or minimizes residual physical disabilities of the patient
  • Returns the individual to optimum and productive level within patient’s capabilities
  • Participates with all other home care personnel in patient care planning
  • Reviews initial history and reviews initial evaluation
  • Consults with physicians regarding patient’s progress
  • Prepares clinical and progress notes
  • Helps communicate the plan of care
  • Consults with family and agency personnel
  • Ability to do extensive bending, lifting and standing on a regular basis
Qualifications
  • A graduate of a PTA Program recognized by the American Physical Therapy Association or other National accepted Council or Association
  • Active license (PTA)
  • Must pass criminal background check
  • Must have current CPR certification or willing to complete
  • Must have active driver's license and clear driving record
  • Computer skills
  • Ability to exercise initiative and independent judgment
